MINUTES
I. CALL TO ORDER
Chair Jim Kelly called the meeting to order at 10:00 a.m. in Conference Room 2
located on the second floor of Auburn City Hall, 25 West Main Street in Auburn.
Members present included Chair Jim Kelly, Chair Pro Tem Bill Petersen,
Member Bob Baggett, Member Larry Brown and Member-at-large Stan Laatsch.
Also present were Board Attorney Steve Gross and City Clerk Shawn Campbell.
II. AGENDA MODIFICATIONS
Claims for LEOFF 1 Members 363477, 165808 were added to the agenda under
Items V.C.4. and V.C.5 respectively.
III. APPROVAL OF MINUTES
A. Minutes of the September 4, 2018 LEOFF Board Meeting
Motion by Chair Pro Tem Petersen, second by Member Laatsch to
approve the minutes as distributed.
MOTION CARRIED UNANIMOUSLY. 5-0
IV. UNFINISHED BUSINESS
A. Pending Disability Leave/Retirement Applications
There was no pending disability leave/retirement application.
B. Pending Medical/HearingNision/Dental Claims
There was no pending claim.
V. NEW BUSINESS
A. Disability Leave/Retirement Applications
No new application for disability leave was received.
B. Appeals/Requests for Reconsideration
No request for reconsideration was received.

C. Medical/Hearing/Vision/Dental Claims
1. LEOFF 1 Member#405444*
Request for reimbursement of vision expenses in the
amount of $389.98 for services received on October 16,
2018. The LEOFF member has not received vision expense
reimbursement in 2018.
Motion by Member Baggett, second by Member Laastch to
approve the claim in the amount of$389.98.
MOTION CARRIED UNANIMOUSLY. 5-0
2. LEOFF 1 Member#937035*
a. Request for reimbursement of vision expenses in the
amount of $199.98 for services received on October 24,
2018. . The LEOFF member has not received vision
expense reimbursement in 2018.
Motion by Chair Pro Tern Petersen, second by Member
Brown to approve the claims a, b and c in the amount of
$199.98. (Member Laastch abstained)
MOTION CARRIED UNANIMOUSLY. 4-0
b. Request for reimbursement of dental expenses in the
amount of $440.00 for services received on October 30,
2018. . The LEOFF member has not received dental
expense reimbursement in 2018.

Request for reimbursement of dental expenses in the amount of
$678.20 for services received on September 11, 2018 and October
2, 2018. The LEOFF 1 member's dental expenses have previously
been determined to be medically necessary.
Motion by Member Baggett, second by Member Brown to approve
the claim in the amount of$678.20. Chair

$105.00 for services received on October 29, 2018.

Request for reimbursement of medical expenses in the amount of
$140.00 for services received on July 20, 2018.
Motion by Member Baggett Chair Pro Tem Petersen second by
Member Brown to table the claim and request member provide an
explanation of benefits from insurance.
MOTION CARRIED UNANIMOUSLY. 5-0
D. Other Discussion
There was no other discussion.
VI. ADJOURNMENT
There being no further business to come before the Board, the meeting
adjourned at 10:08 a.m.
