MINUTES
I. CALL TO ORDER
Chair Jim Kelly called the meeting to order at 10:00 a.m. in Conference Room 2
located on the second floor of Auburn City Hall, 25 West Main Street in Auburn.
Members present included Chair Jim Kelly, Chair Pro Tem Bill Petersen,
Member Bob Baggett, and Member-at-large Stan Laatsch. Member Larry Brown
was excused. Also present were Board Attorney Doug Ruth and Board
Secretary Danielle Daskam.
II. AGENDA MODIFICATIONS
Claims for LEOFF 1 Members 230947, 310533 and 190365 were added to the
agenda under Items IV.C.3, 4 and 5.
III. APPROVAL OF MINUTES
A. Minutes of the July 3, 2018 LEOFF Board Meeting
Motion by Chair Pro Tern Petersen, second by Member Baggett to
approve the minutes as distributed.
MOTION CARRIED UNANIMOUSLY. 4-0
IV. UNFINISHED BUSINESS
A. Pending Disability Leave/Retirement.Applications
There was no pending disability leave/retirement application.
B. Pending Medical/HearingNision/Dental Claims
There was no pending claim.
V. NEW BUSINESS
A. Disability Leave/Retirement Applications
No new application for disability leave was received.
B. Appeals/Requests for Reconsideration.
No request for reconsideration was received.

Request for reimbursement of dental expenses in the amount of
$273.60 for services received on May 31 and July 3, 2018.

Motion by Member Baggett, second by Chair Pro Tern Petersen to
approve the claim in the amount of$273.60.
MOTION CARRIED UNANIMOUSLY. 4-0
2. LEOFF 1 Member#190365
Request for reimbursement of dental expenses in the amount of
$61.00 for services received on July 9, 2018. The retirees dental
expenses have been previously found to be medically necessary.
Motion by Chair Pro Tern Petersen, second by Member Laatsch to
deny the claim.
Members noted the claim for payment form described the services
as dental services while the supporting documentation was for
vision services.
MOTION CARRIED UNANIMOUSLY. 4-0

Request for reimbursement of dental expenses in the amount of
$101.23 for services received on July 31, 2018.

Motion by Chair Pro Tern Petersen, second by Member Baggett to
approve the claim in the amount of$101.23.
MOTION CARRIED UNANIMOUSLY. 4-0.
4. LEOFF 1 Member#310533
Request for reimbursement of dental expenses in the amount of
$88.00 for services received on August 31, 2018. The LEOFF 1
member's dental expenses were previously determined to be
medically necessary.
Motion by Chair Pro Tem Petersen, second by Member Baggett to
approve the claim in the amount of$88.00.
MOTION CARRIED UNANIMOUSLY. 4-0
5. LEOFF 1 Member 190365
Request for reimbursement of dental expenses in the amount of
$99.75 for services received on July 31, 2018. The LEOFF 1
member's dental expenses were previously determined to be
medically necessary.
Motion by Chair Pro Tern Petersen, second by Member Laatsch to
approve the claim in the amount of$99.75.
MOTION CARRIED UNANIMOUSLY. 4-0
D. Other Discussion
Board members acknowledged Board Secretary Daskam, who will be
retiring at the end of the month, and thanked her for her years of service
to the Board.
VI. ADJOURNMENT
There being no further business to come before the Board, the meeting
adjourned at 10:04 a.m.
