MINUTES
I. CALL TO ORDER
R
Chair Jim Kelly called the meeting to order at 10:00 a.m. in Conference Room 2
located on the second floor of Auburn City Hall, 25 West Main Street in Auburn.
Members present included Chair Jim Kelly, Chair Pro Tern Bill Petersen,
Member Bob Baggett, Member Larry Brown and Member-at-large Stan Laatsch.
Also present were Board Attorney Steve Gross and City Clerk Shawn Campbell.
II. AGENDA MODIFICATIONS
Claims for LEOFF 1 Members 165508, 635512, 392099 were added to the
agenda under Items IV.B, V.C.6 and V.C.7 respectively.
III. APPROVAL OF MINUTES
A. Minutes of the November 6, 2018 LEOFF Board Meeting
Motion by Chair Pro Tern Petersen, second by Member Baggett to
approve the minutes as distributed.
MOTION CARRIED UNANIMOUSLY. 5-0
IV. UNFINISHED BUSINESS
A. Pending Disability Leave/Retirement_Applications
There was no pending disability leave/retirement application.
B. Pending Medical/Hearing/Vision/Dental Claims
1. LEOFF 1 Member#165508*
Request for reimbursement of medical expenses in the
amount of $140.88 for services received on July 20, 2018.
Member has provided the Insurance Explanation of Benefits.
The Board discussed coverage of the shingles vaccine.
Motion by Member Baggett, second by Chair Pro Tern
Petersen to approve the claim in the amount of$140.88.

A. Disability Leave/Retirement Applications
No new application for disability leave was received.
B. Appeals/Requests for Reconsideration
No request for reconsideration was received.
C. Medical/Hearinq/Vision/Dental_Claims
1. LEOFF 1 Member#193495*
Request for reimbursement of parking expenses in the amount of
$6.00, plus 26 miles for round trip for services received on
November 19, 2018.
The Board discussed reimbursement of travel expenses. Chair
Kelly noted the Board has not approved travel expenses in the
past.
Motion by Member Baggett, second by Member Laatsch to deny
the claim.
MOTION CARRIED UNANIMOUSLY. 5-0
2. LEOFF 1 Member#937035*
Request for reimbursement of dental expenses in the amount of
$159.90 for services received on November 6, 2018. The LEOFF 1
member's dental expenses have previously been determined to be
medically necessary.
Motion by Member Baggett , second by Chair Pro Tern Petersen to
approve the claim in the amount of$159.90.
MOTION CARRIED UNANIMOUSLY. 5-0
3. LEOFF 1 Member#779147*
Request for reimbursement of dental expenses in the amount of
$171.00 for services received on November 5, 2018.
Motion by Chair Pro Tern Petersen, second by Member Laatsch to
approve the claim in the amount of$171.00.

Request for reimbursement of dental expenses in the amount of
$395.00 for services received on October 16, 2018. The member
has not received dental services in 2018
Motion by Member Laatsch, second by Member Brown to approve
the claim in the amount of$395.00.
MOTION CARRIED UNANIMOUSLY. 5-0
5. LEOFF 1 Member#387501*
A. Request for reimbursement of dental expenses in the amount of
$1,385.00 for services received on June 20, 2018. The LEOFF
1 member's dental expenses have previously been determined
to be medically necessary.
Motion by Member Baggett, second by Chair Pro Tern Petersen
to approve the claim in the amount of$1,385.00.
MOTION CARRIED UNANIMOUSLY. 5-0
B. Request for reimbursement of medical expenses in the amount
of $300.00 for services received between April 19, 2016 and
October 29, 2016.
Chair Kelly stated the dates of service is outside the typically
covered date range. He stated the member has requested
consideration due to not having access to the records.
Motion by Chair Pro Tern Petersen, second by Member Baggett
to approve the claim in the amount of$300.00.
MOTION CARRIED UNANIMOUSLY. 5-0
C. Request for reimbursement of prescription expenses in the
amount of $90.55 for services received between April 19, 2016
and October 29, 2016.
Motion by Chair Pro Tern Petersen, second by Member Brown
to approve the claim in the amount of$90.55.

Request for reimbursement of dental expenses in the amount of
$102.60 for services received on November 28, 2018.
Motion by Chair Pro Tem Petersen, second by Member Brown to
approve the claim in the amount of$102.60.
MOTION CARRIED UNANIMOUSLY. 5-0
7. LEOFF 1 Member#392099*
Request for reimbursement of vision expenses in the amount of
$99.50 for services received on December 3, 2018.
Motion by Member Baggett, second by Member Laatsch approve
the claim in the amount of$99.50.
MOTION CARRIED UNANIMOUSLY. 4-0, (Chair Pro Tern
Petersen obtained)
D. Other Discussion.
1. LEOFF Board Fire Representative
Secretary Campbell reported that nominations for the Fire LEOFF
Board Representative closed November 1, 2018. One nomination
was received, and it was from the current Fire LEOFF Board
representative, Bill Peterson. In accordance with the LEOFF
Policies and Procedures, Bill Peterson is elected to a new term as
Fire LEOFF Representative from January 1, 2019 to December 31,
2020.
2. LEOFF Board Coverage of Vaccines
Chair Pro Tern Petersen questioned if vaccines should be covered
as a medical expense. Chair Kelly stated the vaccine is over the
counter but must still be administered by a pharmacist or
physician.

VI. ADJOURNMENT
There being no further business to come before the Board, the meeting
