MINUTES
I. CALL TO ORDER
Chair Jim Kelly called the meeting to order at 10:00 AM in the Mayor's Office
located at Auburn City Hall, 25 West Main Street, Auburn, WA. Members present
included: Chair Jim Kelly, Jeanne Barber, and Russ Vandver. Member Pete
Lewis arrived at 10:23 AM. Chair Pro Tem Bud Larson was excused from the
meeting. Others present included: Associate City Attorney Karen Gulliver,
Assistant Personnel Director Roger Cartwright, and LEOFF Board Secretary
Danielle Daskam
II. APPROVAL OF MINUTES
A. Minutes of the June 5, 2000 LEOFF Board Meeting
Motion by Member Barber, second by Member Vandver to approve the
June 5, 2000 minutes as distributed.
MOTION CARRIED UNANIMOUSLY. 3-0.
III. UNFINISHED BUSINESS
A. Pending Disability Leave/Retirement Applications
1. Rudy V. Peden
Effective date of disability leave: April 7, 2000
Last day of service: April 6, 2000
On May 1, 2000, the Board approved disability leave effective April
7, 2000 with last day of service on April 6, 2000. On June 5, 2000
the Board accepted the required monthly physician reports and
approved disability leave for another thirty days or until cessation of
disability, whichever occurs first.
A physician's report dated June 20, 2000 was received on July 3,
2000 and distributed to the members at the meeting.
Motion by Member Vandver, second by Member Barber to accept
the physician's report and continue Mr. Peden's disability leave for

an additional thirty (30) days or until cessation of disability,

IV. NEW BUSINESS
A. Disability Leave/Retirement Applications
There was no new application for disability leave/retirement.
B. Medical Claims

The Board reviewed the following claims:
a) Regence Explanation of Benefits Claim No. 0014300773700
processed on June 7, 2000 for medical services received on
May 15, 2000 in the amount of $18.49.
b) Regence Explanation of Benefits Claim No. 0013734370700
processed on May 27, 2000 for medical services received on
February 4 and 7, 2000 in the amount of $20.00.
c) Regence Explanation of Benefits Claim No. 0013734370500
processed on May 20, 2000 for medical services received on
February 7 and 9, 2000 in the amount of $10.00.
Motion by Member Barber, second by Member Vandver to approve
Claim No. 0014300773700 for medical services in the amount of
$18.49.
MOTION CARRIED UNANIMOUSLY. 3-0.
Motion by Member Barber, second by Member Vandver to approve
Claim Nos. 0013734370700 and 0013734370500 in the total
amount of $30.00.

The Board reviewed the following claims:
a) Regence Explanation of Benefits Claim No. 0006000666000
processed on March 15, 2000 for medical services received
on February 7, 2000 in the amount of $9.76.
b) Regence Explanation of Benefits Claim No. 0012610554900
processed on June 7, 2000 for medical services received on
April 27, 2000 in the amount of $8.16.
c) Regence Explanation of Benefits Claim No. 0005934358500
processed on March 27, 2000 for medical services received
on February 21, 2000 in the amount of $15.00.
Motion by Member Vandver, second by Member Barber to deny
Claim No. 0006000666000 in the amount of $9.76 as the claim falls
outside the ninety-day limit for filing claims.
MOTION CARRIED UNANIMOUSLY. 3-0.
Motion by Member Vandver, second by Member Barber to table
Claim Nos. 0012610554900 and 0005934358500 and request Mr.
Wickstrom provide additional documentation supporting the
medical necessity of the medical service.
MOTION CARRIED UNANIMOUSLY. 3-0.
3. Gerry E. Fuller
Request for reimbursement of prescription co-pays, processed
beyond the ninety-day limitation for filing claims, in the amount of
$212.00. ($194.00 in pharmacy expenses within the 90-day
limitation were already processed for payment.)
Motion by Member Barber, second by Member Vandver to deny Mr.
Fuller's claim for reimbursement for pharmacy expenses in the
amount of $212.00 as the claims are outside the ninety-day limit for
filing claims.
MOTION CARRIED UNANIMOUSLY. 3-0.

4. Ed Henderson
The Board reviewed the following claims:
a) Request for reimbursement of prescription co-pays (some
beyond the ninety-day limitation for filing claims) in the total
amount of $91.50.
b) Request for reimbursement of vision exam deductible in the
amount of $10.00.
Chair Kelly noted that Mr. Henderson included a notation of
"Duplicate" next to some of the claims. Board Secretary Daskam
explained that during the first week of May 2000, Mr. Henderson
telephoned inquiring about the status of his claim, which he said he
had mailed several weeks before. She explained to Mr. Henderson
that there was no record of receiving a claim from him since before
January 2000. Member Vandver verified that Board Secretary
Daskam had called him after Mr. Henderson's telephone call,
inquiring whether Mr. Vandver recalled receiving a claim from Mr.
Henderson. No claim had been received from Mr. Henderson. The
claim before the Board includes duplicates of those claims
previously mailed by Mr. Henderson, but not received by the
LEOFF Board.
Motion by Member Vandver, second by Member Barber to waive
the ninety-day limitation for Mr. Henderson's claims in light of the
explanation regarding the delay in receiving them and approve Mr.
Henderson's claims for pharmacy expenses in the amount of
$91.50 and vision exam deductible in the amount of $10.00.

Motion by Member Vandver, second by Member Barber to approve
Mr. Cadwell's claims with date processed from March 29, 2000 to
May 24, 2000 in the total amount of $184.09 and deny Claim No.
0006331102900 in the amount of $16.87 as it falls outside the
ninety-day limit for filing claims.
MOTION CARRIED UNANIMOUSLY. 3-0.
Member Lewis arrived at this time.
The Board discussed Mr. Caldwell's letter dated June 26, 2000 and
the accompanying letter from his physician, Dr. Dennis Frederick.
In his letter, Mr. Caldwell requests direction from the Board
regarding Dr. Frederick's announcement that he is withdrawing as
a Regence preferred provider. The Board discussed the Board's
options and obligations regarding medical expenses for LEOFF
members and the number of physicians/providers who are
withdrawing from Regence.

Chair Kelly instructed the Board Secretary to correspond with Mr.
Caldwell to explain that the Board will continue to follow the LEOFF
Board Policies and Procedures and state law, and the Board will
continue to reimburse members' claims according to Board policy
regardless of whether his physician is a preferred provider under
the Regence health plan.
6. Patricia Djarf
The Board reviewed the following claims:
a) Request for reimbursement of mail order and local pharmacy
expenses (some outside the ninety-day limitation) in the total
amount of $141.26.
b) 1999 Form SSA-1099 Benefit Statement for reimbursement
of Medicare premiums paid in the amount of $546.00.
Motion by Member Lewis, second by Member Barber to approve
Ms. Djarfs claim for reimbursement of 1999 Medicare premiums in
the amount of $546.00.
MOTION CARRIED UNANIMOUSLY. 4-0.
Motion by Member Barber, second by Member Lewis to approve
Ms. Djarf's pharmacy expense claims within the ninety-day limit
($56.00) and deny the pharmacy claims. outside the ninety-day limit
($85.26.)
MOTION CARlED UNANIMOUSLY. 4-0.
C. Correspondence
1. June 5, 2000 correspondence from the State Department of
Retirement Systems transmitting revised LEOFF Plan 1 Local
Disability Board Guidelines.
Chair Kelly acknowledged receipt of the revised LEOFF Plan 1
Local Disability Board Guidelines received from the State
Department of Retirement Systems.
