MINUTES
CALL TO ORDER
Chair Jim Kelly called the meeting to order at 10:00 a.m. in Conference Room 2
located on the second floor of Auburn City Hall, 25 West Main Street in Auburn.
Members present included Chair Jim Kelly, Chair Pro Tern Bill Petersen,
Member Bob Baggett and Member-at-large Stan Laatsch. Member Larry Brown
was excused.
Also present Human Resources Analyst & Board Secretary Terry Mendoza and
City Clerk Shawn Campbell.
II. AGENDA MODIFICATIONS
There was no agenda modification.
III. APPROVAL OF MINUTES
A. Minutes of the April 2, 2019 LEOFF Board Meeting
Motion by Member Baggett, seconded by Chair Pro Tern Petersen to
approve the minutes as distributed.
MOTION CARRIED UNANIMOUSLY. 4-0
IV. UNFINISHED BUSINESS
A. Pending Disability Leave/Retirement Applications
There was no pending disability leave/retirement application.
B. Pending Medical/HearingNision/Dental Claims
LEOFF 1 Member#284134*
Request for reimbursement of prescription expenses in the amount
of $1,117.29 for services received between December of 2017 to
December of 2018. No new information was provided.
Motion by Chair Pro Tern Petersen to table the request until
additional information is provided. Seconded by Member Baggett.

Chair Kelly advised that Mr. Huseby corresponded to the Board
Secretary indicating that he was not going to be appealing the
denial for the Long Term Care expenses.
V. NEW BUSINESS
A. Disability Leave/Retirement Applications
No new application for disability leave was received.
B. Appeals/Requests for Reconsideration
No request for reconsideration was received.
C. Medical/HearingNision/Dental Claims
1. LEOFF 1 Member#444135*
a. Request for reimbursement of dental expenses in the amount of
$145.00 for services received on April 14, 2019. The LEOFF
member has received no dental reimbursements in 2019.
Motion by Member Baggett to approve claim. Seconded by
Member Laatsch.
MOTION CARRIED UNANIMOUSLY. 4-0
b. Request for approval/reimbursement of denture expenses in the
amount of $2,650.00 for services received on April 15, 2019.
The LEOFF member received pre-approval for these expenses.
Motion by Chair Pro Tem Petersen to approve payment up to
the policy limit. Seconded by Member Baggett.
MOTION CARRIED UNANIMOUSLY. 4-0
2. LEOFF 1 Member#642873*
Request for reimbursement of dental expenses in the amount of
$214.00 for services received on April 10, 2019. The LEOFF
member has received no dental reimbursements in 2019.
Motion by Member Laatsch to approve claim. Seconded by Chair
Pro Tern Petersen.

Request for reimbursement of medical transportation expenses in
the amount of $455.00 for services received on four separate
occasions in November of 2018.
Motion by Chair Pro Tem Petersen to table the request until
additional information is provided specific to the medical necessity
for the transportation services. Seconded by Member Baggett.
MOTION CARRIED UNANIMOUSLY. 4-0
4. LEOFF 1 Member#629199*
Request for reimbursement of dental expenses in the amount of
$177.00 for services received on May 7, 2019. The LEOFF
Members dental expenses were previously found to be medically
necessary.
Motion by Member Baggett to approve claim. Seconded by Chair
Pro Tem Petersen.
MOTION CARRIED UNANIMOUSLY. 4-0
5. LEOFF 1 Member#948705*
Request for reimbursement of dental expenses in the amount of
$312.00 for services received on May 2, 2019. The LEOFF
member has received no dental reimbursements in 2019.
Motion by Member Laastch to approve claim. Seconded by Chair
Pro Tern Petersen.
MOTION CARRIED UNANIMOUSLY. 4-0
D. Other Discussion
a. Revised Draft Medical Claim Form.
The Board discussed changes to the Medical Claim form and
requested to have this item on the June meeting agenda.
b. LEOFF Board Conference Update
Chair Pro Tern Petersen and Member Baggett shared information
from the annual LEOFF Conference held in Chelan.

VI. ADJOURNMENT
There being no further business to come before the Board, the meeting
