MINUTES
I. CALL TO ORDER
Chair Jim Kelly called the meeting to order at 10:00 a.m. in Conference Room 2
located on the second floor of Auburn City Hall, 25 West Main Street in Auburn.
Members present included Chair Pro Tern Bill Petersen and Member-at-large
Stan Laatsch. Also present was Board Secretary Terry Mendoza and Board
Attorney Steve Gross. Member Bob Baggett and Member Larry Brown were
excused.

III. APPROVAL OF MINUTES
A. Minutes of the July 2, 2019 LEOFF Board Meeting*
Motion by Chair Pro Tem Petersen, seconded by Member Laatsch to
approve the minutes as distributed.
MOTION CARRIED UNANIMOUSLY. 3-0
IV. UNFINISHED BUSINESS
A. Pending Medical/HearingNision/Dental Claims
1. LEOFF 1 Member#857532*
Request for reimbursement of medical transportation expenses in
the amount of $455.00 for services received on four separate
occasions in November of 2018. Item was tabled from the June 4,
2019 meeting. Item tabled from the July 2, 2019 meeting.
Motion by Chair Pro Tem Petersen to approve claim as submitted.
Second by Member Laatsh.

Request for reimbursement of medical expenses in the amount of
$99.00 for services received in June 2019. Item tabled from the
July 2, 2019 meeting.

Chair Kelly informed the Board that Member #215086 withdrew his
claim. No action required.
3. LEOFF 1 Member#363477*
Request for reimbursement of medical equipment in the amount of
$30.00 purchased on April 29, 2019. Item tabled from the July 2,
2019 meeting.
Motion by Chair Pro Tem Petersen to table the request until an
explanation of benefits is submitted. Seconded by Member
Laatsch. *Note: Secretary Mendoza informed the Board that there is a
change in the Power of Attorney for Member #363477. The new POA is
working on obtaining the required documentation requested by the Board.

4. LEOFF 1 Member#321061*
Request for reimbursement of medical expenses in the amount of
$100.00 for services received in May 2019. Item tabled from the
July 2, 2019 meeting.
Motion by Chair Pro Tem Petersen to table the request until an
explanation of benefits is submitted. Seconded by Member
Laatsch.
MOTION CARRIED UNANIMOUSLY. 3-0
5. LEOFF 1 Member#937035*
Request for reimbursement of dental expenses in the amount of
$230.00 for services received on May 15, 2019. The LEOFF
member's dental expenses were previously found to be medically
necessary. Item postponed from the July 2, 2019 meeting.
Item postponed until the September LEOFF Board meeting in order
to have a quorum to take action.
6. LEOFF 1 Member#392099*
Request for reimbursement of vision expenses in the amount of
$108.54 for services received on June 14, 2019. The LEOFF
member has received no reimbursement for vision expenses in
2019. Item postponed from the July 2, 2019 meeting.
Item postponed until the September LEOFF Board meeting in order
to have a quorum to take action.

a. Request for reimbursement of dental expenses in the
amount of $368.00 for services received July 2019. The
LEOFF member has not received any dental reimbursement
in 2019.
Motion by Chair Pro Tem Petersen to approve claim as
submitted. Second by Member Laatsh.

b. Request for reimbursement of vision expenses in the
amount of $159.97 purchased on July 2, 2019. The LEOFF
member has received no reimbursement for vision expenses
in 2019.
Motion by Member Laatsch to approve claim as submitted.

amount of $437.00 for services received in December 2018
and May 2019. The LEOFF member's dental expenses were
previously found to be medically necessary. The LEOFF
member received no dental reimbursement in 2018 and
2019.

submitted. Second by Member Laatsh.

reimbursement in 2019.

Request for reimbursement of dental expenses in the amount of
$493.95 for services received in May and August of 2018. The
LEOFF member did not receive any dental reimbursements in
2018. *Note: Chair Kelly informed the Board that he spoke with the
member regarding the timeliness of this claim. The member was delayed
in providing this information to the Board due to other significant health
issues in 2018.
Motion by Chair Pro Tem Petersen to approve claim as submitted.

Request Board approve dental procedures as a medical necessity
per Doctor's documentation dated July 1, 2019.
Motion by Chair Pro Tern Petersen to accept the documentation
establishing the medical necessity for dental procedures. Second
by Member Laatsh.
MOTION CARRIED UNAIMOUSLY. 3-0
C. Other Discussion
Chair Kelly informed the Board that he spoke with Secretary Mendoza
about dental procedures being approved as medically necessary. The
underlying question was if a member's dental procedure/treatment has
been approved as medically necessary has the Board required the
member to submit those dental procedure/treatment costs to their medical
insurance provider. Secretary Mendoza has already initiated an inquiry
with both United Health Care and Premera confirming if coverage would
apply in these types of situations. The item will be added to a future
agenda once information has been received.
VI. ADJOURNMENT
There being no further business to come before the Board, the meeting
adjourned at 10:21 a.m.
