MINUTES
I. CALL TO ORDER
Chair Jim Kelly called the meeting to order at 10:00 a.m. via our Zoom
Videoconferencing call.
Members present on the Zoom Videoconferencing call included Chair Jim Kelly,
Chair Pro Tem Bill Petersen, Member Stan Laatsch, Member Bob Baggett, and
Member Chris Stearns. Also present was Board Secretary Terry Mendoza and
Assistant City Attorney Doug Ruth.
II. AGENDA MODIFICATIONS
There were no agenda modifications.
III. APPROVAL OF MINUTES
A. Minutes of the July 7, 2020 LEOFF Board Meeting*
Motion by Member Baggett, seconded by Chair Pro Tern Petersen to approve
the minutes as published.
MOTION CARRIED UNANIMOUSLY. 5-0
IV. UNFINISHED BUSINESS
A. Pending Medical/Hearing/Vision/Dental Claims
1. LEOFF 1 Member#562217*
Request for reimbursement of end of life/long term care expenses in
the amount of $10,350.00 for services received between May 13 and
25, 2020. The LEOFF member was not covered by any long-term
care plan. This item was tabled from the July 7, 2020, LEOFF Board
Meeting.
The Board received additional information requested for the claim.

submitted. Second by Member Laatsch.

V. NEW BUSINESS
A. Appeals/Requests for Reconsideration
No appeal/request for reconsideration has been received.
B. Medical/Hearing/Vision/Dental Claims

a. Request for reimbursement of medical expenses in the amount
of$744.60 for long-term care services received between April 1
through April 30, 2020.
Motion by Chair Pro Tern Petersen to table the claim for
further clarification/identification of the determined
reimbursement amount requested. Second by Member
Laatsch.
MOTION CARRIED UNANIMOUSLY. 5-0
b. Request for reimbursement of medical expenses in the amount
of $1,111.12 for long-term care services received between May
1 through 31, 2020.
Motion by Chair Pro Tern Petersen to table the claim for
further clarification/identification of the determined
reimbursement amount requested. Second by Member
Baggett.
MOTION CARRIED UNANIMOUSLY. 5-0
2. LEOFF 1 Member#779147*
a. Request for reimbursement of dental expenses in the amount
of $299.25 for services received July 2, 2020. The LEOFF
member has not received dental reimbursement in 2020.
Motion by Member Baggett to approve the claim as submitted.

member has not received dental reimbursement in 2020.
Motion by Chair Pro Tem Petersen to approve the claim as
submitted. Second by Member Laatsch.

Request for reimbursement of dental expenses in the amount of
$2,575.80 for services received on June 23 and July 21, 2020. The
LEOFF member's dental expenses were previously found to be
medically necessary. The LEOFF member has received dental
reimbursement in 2020.

Request for reimbursement of dental expenses in the amount of
$167.40 for services received on July 23, 2020. The LEOFF
member's dental expenses were previously found to be medically
necessary. The LEOFF member has not received dental
reimbursement in 2020.

$413.25 for services received on June 12, 2020. The LEOFF
member's dental expenses were previously found to be medically
necessary. The LEOFF member has not received dental
reimbursement in 2020.
Member Laatsch recused himself from this item.
Motion by Chair Pro Tern Petersen to approve the claim as
submitted. Second by Member Baggett.
MOTION CARRIED UNANIMOUSLY. 4-0
C. Other Discussion
Chair Kelly expressed his appreciation for Secretary Mendoza delivering the
agenda packets in-person during the pandemic and for providing an
advanced copy of the updated Board Policies and Procedures.
VI. ADJOURNMENT
There being no further business to come before the Board, the meeting
adjourned at 10:25 a.m.
