MINUTES
I. CALL TO ORDER
Chair Jim Kelly called the meeting to order at 10:01 a.m. via our Zoom
Videoconferencing call.
Members present on the Zoom Videoconferencing call included Chair Jim Kelly,
Chair Pro Tem Bill Petersen, Member Stan Laatsch, Member Bob Baggett, and
Member Chris Stearns. Also present was Board Secretary Terry Mendoza and
Assistant City Attorney Doug Ruth.

III. APPROVAL OF MINUTES
A. Minutes of the March 3, 2020 LEOFF Board Meeting*
Motion by Member Baggett, seconded by Chair Pro Tem Petersen to
approve the minutes as published.
MOTION CARRIED UNANIMOUSLY. 5-0
IV. UNFINISHED BUSINESS
A. Pending Medical/Hearing/Vision/Dental Claims
1. LEOFF 1 Member#490024*
Request for reimbursement of medical expenses in the amount of
$1 ,059.48 for services received between November 2019 through
January 2020. This item was tabled from the March 3rd meeting.
No additional information was received for this claim.
Motion by Chair Pro Tem Petersen to table the claim until an
explanation of benefits is submitted. Seconded by Member
Baggett.
MOTION CARRIED UNANIMOUSLY. 5-0
V. NEW BUSINESS

No appeal/request for reconsideration has been received.

Request for reimbursement of dental expenses in the amount of
$90.00 for services received on February 3, 2020. The LEOFF
member has not received dental reimbursement in 2020.
Motion by Member Baggett to approve the claim as submitted.
Second by Member Stearns.

Request for reimbursement of medical expenses in the amount of
$3,056.00 for home care services received between February 4th
through 8th, 2020.
Motion by Chair Pro Tem Petersen to table the claim until
additional information is submitted. Seconded by Member Laatsch.
MOTION CARRIED UNANIMOUSLY. 5-0
3. LEOFF 1 Member#310533*
Request for reimbursement of dental expenses in the amount of
$178.00 for services received on March 17, 2020. The LEOFF
member has not received dental reimbursement in 2020. The
LEOFF member's dental expenses were previously found to be
medically necessary.
Motion by Member Baggett to approve the claim as submitted.
Second by Chair Pro Tern Petersen.

Request for reimbursement of medical expenses in the amount of
$2,723.88 for the purchase of hearing aids on March 7th, 2020.
Motion by Chair Pro Tern Petersen to table the claim until

Motion by Member Laatsch to approve the claim as submitted.

Request for reimbursement of medical expenses in the amount of
$1,188.00 for services received on February 4th and 6th, 2020.
Motion by Chair Pro Tern Petersen to approve the claim as
submitted. Second by Member Baggett.
MOTION CARRIED UNANIMOUSLY. 5-0
7. LEOFF 1 Member#970468*
Request for reimbursement of dental expenses in the amount of
$163.20 for services received on March 10th, 2020. The LEOFF
member has not received dental reimbursement in 2020. The
LEOFF member's dental expenses were previously found to be
medically necessary.
Chair Kelly recused himself from this item.
Motion by Member Baggett to approve the claim as submitted.
Second by Member Laatsch.
MOTION CARRIED UNANIMOUSLY. 4-0
C. Other Discussion
1. Policy Review
In light of the current COVID-19 situation affecting our region, Chair
Kelly recommended that the policy review discussion be continued
until such time when the board can meet in person.
VI. ADJOURNMENT
There being no further business to come before the Board, the meeting
adjourned at 10:23 a.m.
