MINUTES
I. CALL TO ORDER
Chair Jim Kelly called the meeting to order at 10:00 a.m. via our Zoom
videoconferencing call.
Members present on the Zoom call included Chair Jim Kelly, Chair Pro Tem Bill
Petersen, Member Stan Laatsch, Member Bob Baggett, and Member Chris Stearns.
Also present was Board Secretary Terry Mendoza and Senior City Staff Attorney
Doug Ruth.
Member Stearns joined the meeting at 10:03 a.m.
II. AGENDA MODIFICATIONS
Claim for LEOFF Member #937035 was added to the agenda under Item V.B.12.
III. APPROVAL OF MINUTES
A. Minutes of the April 6, 2021 LEOFF Board Meeting*
Motion by Chair Pro Tem Petersen to approve the minutes as published.
Second by Member Laatsch.

Motion by Member Baggett to approve the claim as submitted. Second
by Chair Pro Tem Petersen.

Request for reimbursement of dental expenses in the amount of
$108.00 for services received on April 8, 2021.
Motion by Chair Pro Tem Petersen to approve the claim as submitted.

Request for reimbursement of medical expenses in the amount of
$1,000.00 for the purchase of medical equipment on October 1, 2020.
Motion by Chair Pro Tem Petersen to table the claim until a letter of
medical necessity is provided by the prescribing physician. Second by
Member Baggett.

Request for reimbursement of dental expenses in the amount of
$2,309.50 for services received on February 15, February 17, and
March 12, 2021.
Motion by Member Baggett to table the claim for clarification from the
LEOFF Member on how the requested amount for reimbursement was
determined. Second by Member Laatsch.

Motion by Member Baggett to approve the claim as submitted. Second
by Chair Pro Tem Petersen.
Board discussed the unknown accessory item. Board Secretary will
notify the LEOFF Member regarding the requested reimbursement of
the accessory item.
Motion by Member Baggett to amend approval of the claim minus the
cost for the unknown accessory item. Second by Chair Pro Tem
Petersen.

$320.00 for services received on April 7, 2021.

C. Other Discussion
1. Cost comparison for Hearing Aids
This item was tabled from the April 6. 2021 meeting for additional
information from Costco.
Board discussed Costco requiring membership to use Hearing Aid
benefits and current Board policy. Chair Kelly, City Staff Attorney Ruth,
City of Auburn Law Enforcement Officers and Firefighters Disability Board

and Board Secretary Mendoza will work on an amendment, if
applicable, and forward to a future Board meeting.
VI. ADJOURNMENT
There being no further business to come before the Board, the meeting was adjourned
at 10:31 a.m.
