MINUTES
I. CALL TO ORDER
Chair Jim Kelly called the meeting to order at 10:00 a.m. via our Zoom
videoconferencing call.
Members present on the Zoom call included Chair Pro Tem Bill Petersen, Member Stan
Laatsch, Member Bob Baggett, and Member Chris Stearns. Also present was Sr. City
Staff Attorney Doug Ruth and Board Secretary Terry Mendoza.
Member Chris Stearns joined the meeting at 10:01 a.m.
II. AGENDA MODIFICATIONS
There were no agenda modifications.
III. APPROVAL OF MINUTES
A. Minutes of the September 7, 2021, LEOFF Board Meeting*
Motion by Member Baggett to approve the minutes as published. Second by
Member Laatsch.
MOTION CARRIED UNANIMOUSLY. 4-0

for services received between June 10 and August 27, 2021.
Motion by Chair Pro Tern Petersen to approve the claim as submitted.
Second by Member Baggett.
MOTION CARRIED UNANIMOUSLY. 4-0

Request for reimbursement of medical expenses in the amount of$546.90
for Long Term Care services received July 1 - 31, 2021.
Motion by Chair Pro Tern Petersen to approve the claim for an adjusted
total of $531.90. Second by Member Laatsch.
MOTION CARRIED UNANIMOUSLY. 5-0
Motion by Chair Pro Tern Petersen to table a portion of the claim ($15.00)
until a letter of medical necessity is provided. Second by Member Stearns.

$500.00 for services received September 22, 2021.
Motion by Member Laatsch to approve the claim as submitted. Second

b. Request for reimbursement of medical expenses in the amount of
$75.00 for services received September 23, 2021.
Motion by Member Baggett to approve the claim as submitted. Second
by Member Laatsch.

c. Request for reimbursement of medical expenses in the amount of
$75.00 for services received on September 30, 2021.
Motion by Member Laatsch to approve the claim as submitted. Second
by Chair Pro Tem Petersen.

Board Secretary Mendoza provided a draft of the Police Election
information that would go out to LEOFF 1 and 2 Retired and Active
members. The current Police Representative Board position expires
December 31, 2021 .
2. Member-at-Larqe Election*
Board Secretary Mendoza provided a draft of the publication information
that would be posted regarding the Member-at-large election. The
current position expires December 31, 2021. Board members
discussed and would like the information to be posted in another local
publication as well as on the City's website.
3. Councilmember Representative Appointments
Board Secretary advised that a reminder has been provided to the
mayor on the two Councilmember LEOFF Board positions expiring on
December 31, 2021.
VI. ADJOURNMENT
There being no further business to come before the Board, the meeting was adjourned at
10:13 a.m.
