MINUTES
I. CALL TO ORDER
Chair Kelly called the meeting to order at 10:00 AM in Conference Room 1
located on the main floor of Auburn City Hall, 25 West Main Street, Auburn, WA.
Members present included: Chair Jim Kelly, Chair Pro Tem Bud Larson, and
Member Pete Lewis. Members Jeanne Barber and Russ Vandver were
excused. Also present was Board Secretary Danielle Daskam.
II. APPROVAL OF MINUTES
A. Minutes of the November 6, 2000 LEOFF Board Meeting
Motion by Member Larson, second by Member Lewis to approve the
November 6, 2000 meeting minutes as distributed.
MOTION CARRIED UNANIMOUSLY. 3-0.
III. UNFINISHED BUSINESS
A. Pending Disability Leave/Retirement Applications
There was no pending disability leave/retirement application.
B. Claims
1. Ron Mauel
On September 5, October 2, and November 6, 2000, the LEOFF
Board tabled Mr. Mauel's request for reimbursement of pharmacy
expenses in the amount of $7.00 pending the receipt of the
pharmacy prescription receipt. No new information was received.
a. Pharmacy expense in the amount of $7.00 sold on July 19,
2000.
Motion by Member Larson, second by Member Lewis to reject Mr.
Mauel's claim for lack of information.
MOTION CARRIED UNANIMOUSLY. 3-0.

2. R.K. Calkins
On September 5, October 2, and November 6, 2000, the LEOFF
Board tabled Mr. Calkins' claim pending the receipt of explanation
of benefits documentation from Medicare and Regence. No new
information was received.
a. Request for reimbursement of chiropractic services received
on August 2, 2000 in the amount of $50.00.
Motion by Member Larson, second by Member Lewis to reject Mr.
Calkins claim for lack of information.
MOTION CARRIED UNANIMOUSLY. 3-0.
3. Hans Hansen
On November 6, 2000 the LEOFF Board tabled Mr. Hansen's claim
pending the receipt of Regence Explanation of Benefits Statement.
No new information was received.
a. Request for reimbursement of medical expenses incurred on
July 15, 2000 in the amount of $16.85.
Motion by Member Lewis, second by Member Larson to table Mr.
Hansen's claim for an additional month.
MOTION CARRIED UNANIMOUSLY. 3-0.
IV. NEW BUSINESS
A. Disability Leave/Retirement Applications
There was no new application for disability leave/retirement.
B. Medical Claims

processed on October 18, 2000 for medical services
received on September 21, 2000 in the amount of $15.00.

Motion by Member Larson, second by Member Lewis to approve
Ms. Darough's claim in the amount of $15.00.
MOTION CARRIED UNANIMOUSLY. 3-0.
2. John Wickstrom
a. Regence Explanation of Benefits Claim No. 0030248021800
processed on October 28, 2000 for medical services
received on July 6, 2000 in the amount of $15.00; and
b. Regence Explanation of Benefits Claim No. 0031148012700
processed on November 6, 2000 for medical services
received on October 18 and 20, 2000 in the amount of
$30.00.
Motion by Member Larson, second by Member Lewis to approve
Mr. Wickstrom's claims in the total amount of $45.00.
MOTION CARRIED UNANIMOUSLY. 3-0.
3. Richard Huseby
a. Regence Explanation of Benefits Claim No. 0029810230900
processed on October 24, 2000 and Medicare Explanation
of Benefits Statement dated August 4, 2000 for medical
services received on May 16, 2000 in the amount of
$210.00.
Motion by Member Larson, second by Member Lewis to table Mr.
Huseby's claim pending the receipt of additional documentation to
support the medical necessity of the medical services received on
May 16, 2000.
MOTION CARRIED UNANIMOUSLY. 3-0.
C. Update on Fire Department LEOFF Board Representative Election
According to LEOFF Board Policy and Procedure, 1.05, Elections, the
LEOFF Board Secretary is to prepare and mail forms for nomination of
law enforcement or firefighter representative to each active and retired law
enforcement or fire fighter personnel for a two year term. Fire

Representative Russ Vandver's term expires on December 31, 2000.
Nomination forms were mailed October 12, 2000 and the deadline for
receiving nominations was October 27, 2000 at 5:00 PM. Two
nominations were submitted and election ballots containing the two
nominations of Ed Henderson and Russ Vandver were mailed November
13, 2000. Ballots were due postmarked by November 23, 2000. Ballots
were counted on November 30, 2000. The results are as follows:
Ed Henderson 4
Russ Vandver 43
A total of 47 ballots were received by the November 23, 2000 postmark.
Board members acknowledged the results of the Fire Department
representative election. Russ Vandver is elected as the Fire Department
representative to the LEOFF Board for a two year term beginning January
1, 2001.
V. ADJOURNMENT
There being no further business to come before the Board, the meeting
adjourned at 10:11 AM.
