MINUTES
I. CALL TO ORDER
Chair Kelly called the meeting to order at 10:00 AM in the Council Work Area
located in the Mayor's office at Auburn City Hall, 25 West Main Street, Auburn,
WA. Members present included: Chair Jim Kelly, Member Jeanne Barber,
Member Russ Vandver and Member Stacey Brothers. Chair Pro Tem Bud
Larson was absent. Others present included: Mayor Charles Booth, Personnel
Director Brenda Heineman, and Deputy Board Secretary Cathy Richardson.
II. APPROVAL OF MINUTES
A. Minutes of the January 2, 2001 LEOFF Board Meeting
Motion by Member Barber and second by Member Brothers to approve
the January 2, 2001 minutes as distributed.
MOTION CARRIED UNANIMOUSLY. 4-0.
III. UNFINISHED BUSINESS
A. Pendin~ Disability Leave/Retirement Applications
There were no pending disability leave/retirement applications.
B. Claims
1. Richard Huseby
On December 4, 2000, the LEOFF Board tabled Mr. Huseby's
claim pending the receipt of additional information documenting the
medical necessity of the medical services received. On December
18, 2000, Mr. Huseby submitted a letter from Dr. David Wukasch
regarding Mr. Huseby's medical condition. On January 2, 2001,
the LEOFF Board tabled Mr. Huseby's claim and requested that he
resubmit his claim to Regence with the additional information
provided by his physician. No new information was received.

$210.00.
Chair Kelly noted that no additional information has been
received to date from Mr. Huseby.
Motion by Member Vandver and second by Member
Brothers to table Mr. Huseby's claim for an additional month.
MOTION CARRIED UNANIMOUSLY. 4-0.
2. Hans Hartsen
On January 2, 2001, the LEOFF Board tabled the following claim
pending the receipt of the Regence Explanation of Benefits
Statement. No new information was received.
a. Medicare Claim Number 531147586A processed on
September 16, 2000 for medical services received July 15
through July 22, 2000 in the amount of $776.00.
Chair Kelly noted that no additional information has been
received to date from Mr. Hansen's representative.
Motion by Member Barber and second by Member Vandyer
to table Mr. Hansen's claim for an additional month.
MOTION CARRIED UNANIMOUSLY. 4-0.
IV. NEW BUSINESS
A. Disability Leave/Retirement Applications
There were no new applications for disability leave/retirement.
B. Medical Claims
1. 2000 Form SSA-1099 Benefit Statement for reimbursement of
Medicare premiums paid:
a. John Baker in the amount of $546.00
b. Murray Board in the amount of $546.00
c. Helen Bowden in the amount of $637.00

Chair Kelly stated that upon his review, all claims submitted for
reimbursement for Medicare premiums appear to be in order.
Motion by Member Vandver and second by Member Brothers to
approve reimbursement of Medicare premiums paid under items
4.B.l.a through 4.B.1.1.
MOTION CARRIED UNANIMOUSLY. 4-0
2. Doris Damugh
a. Regence Explanation of Benefits Statement No.
0035600407500 processed on January 10, 2001 for medical
services received on December 5, 2000 in the amount of
$15.00.
Motion by Member Brothers and second by Member Barber
to approve Ms. Darough's claim in the amount of $15.00.
MOTION CARRIED UNANIMOUSLY. 4-0.
3. Raymond E. Sherman
a. Regence Explanation of Benefits Statement No.
0022931407600 processed on August 23, 2000 for medical
services received on June 27, 2000 in the amount of $28.00.
Board members reviewed the claim submitted by Mr.
Sherman. Chair Kelly noted that additional information is
needed to determine the medical necessity of the claim
submitted.

Motion by Member Vandver and second by Member
Brothers to table Mr. Sherman's claim for an additional
month and request additional information from Mr. Sherman.
MOTION CARRIED UNANIMOUSLY. 4-0.
V. ADJOURNMENT
There being no further business to come before the Board, the meeting
adjourned at 10:12 AM.
