CALL TO ORDER Chair Kelly called the meeting to order at 10:00 AM in Conference Room 1 located in Auburn City Hall, 25 West Main Street, Auburn, WA. Members present included Chair Kelly, Chair Pro Tem Bud Larson, and Member Stacey Brothers. Members Jeanne Barber and Russ Vandver were excused. Others present included: Mayor Charles A. Booth, Associate City Attorney Karen Gulliver, Personnel Director Brenda Heineman, and Board Secretary Danielle Daskam. I1. APPROVAL OF MINUTES A. Minutes of the February 5, 2001 LEOFF Board Meeting Motion by Member Brothers, second by Member Larson to approve the February 5, 2001 minutes as distributed. MOTION CARRIED UNANIMOUSLY. 3-0. III. UNFINISHED BUSINESS A. Pending Disability Leave/Retirement Applications There was no pending disability leave/retirement application. B. Claims 1. Richard Huseby On December 4, 2000, the LEOFF Board tabled Mr. Huseby's claim pending the receipt of additional information documenting the medical necessity of the medical services received. On December 18, 2000, Mr. Huseby submitted a letter from Dr. David Wukasch regarding Mr. Huseby's medical condition. On January 2, 2001, the LEOFF Board tabled Mr. Huseby's claim and requested that he resubmit his claim to Regence with the additional information provided by his physician. On February 5, 2001, the Board tabled Mr. Huseby's claim. No new information was received. Regence Explanation of Benefits Claim No. 0029810230900 processed on October 24, 2000 and Medicare Explanation of Benefits Statement dated August 4, 2000 for medical services received on May 16, 2000 in the amount of $210.00. Motion by Member Larson, second by Member Brothers to table Mr. Huseby's claim for an additional month. MOTION CARRIED UNANIMOUSLY. 3-0. 2. Hans Hansen On January 2 and February 5, 2001, the LEOFF Board tabled the following claim pending the receipt of the Regence Explanation of Benefits Statement. No new information was received. a. Medicare Claim Number 531147586A processed on September 16, 2000 for medical services received July 15 through July 22, 2000 in the amount of $776.00. Motion by member Larson, second by Member Brothers to table Mr. Hansen's claim for an additional month. MOTION CARRIED UNANIMOUSLY. 3-0. 3. Raymond E. Sherman On February 5, 2001, the Board tabled Mr. Sherman's claim. a. Regence Explanation of Benefits Statement No. 0022931407600 processed on August 23, 2000 for medical services received on June 27, 2000 in the amount of $28.00. Chair Kelly noted that although Mr. Sherman's claim was tabled at the last meeting, further review of Mr. Sherman's claim revealed the claim was outside the ninety (90) day limit for filing claims. Motion by Member Larson, second by Member Brothers to deny Mr. Sherman's claim in the amount of $28.00 and instruct the Board Secretary to notify Mr. Sherman of the Board's action. IV. NEW BUSINESS A. Disability Leave/Retirement Applications There was no new application for disability leave/retirement. B. Medical Claims 1. Bruce Caldwell a. Regence Explanation of Benefits Statement No. 0034331158000 processed on December 27, 2000, for services received on December 5, 2000, in the amount of $210.00; and Regence Explanation of Benefits Statement No. 0028631210700 processed on October 25, 2000 for services received on October 10; 2000 in the amount of $40.00; for a total claim amount of $250.00. b. Regence Explanation of Benefits Statement No. 0101131213400 processed on January 31, 2001, for services received on January 9, 2001, in the amount of $84.07; and Regence Explanation of Benefits Statement No. 0034931325500 processed on December 27, 2000 for services received on December 12, 2000, in the amount of $59.54, for a total claim amount of $143.61. Board members reviewed Mr. Caldwell's claims and determined that some were outside the ninety (90) day limit for filing claims. Motion by Member Larson, second by Member Brothers to approve claim for payment for Regence Explanation of Benefits Statement No. 0034331158000 in the amount of $210.00. MOTION CARRIED UNANIMOUSLY. 3-0. Motion by Member Larson, second by Member Brothers to deny claim for payment for Regence Explanation of Benefits LEOFF Board Minutes March 5, 2001 Page 3 City of Auburn Law Enforcement Officers and Firefighters Disability Board Monday, March 5, 2001 - 10:00 AM - Conference Room I Statement No. 0028631210700 in the amount of $40.00 as the date processed is outside the ninety day limit for filing claims. MOTION CARRIED UNANIMOUSLY. 3-0. Motion by Member Larson, second by Member Brothers to approve claim for payment for Regence Explanation of Benefits Statement Nos. 0101131213400 and 0034931325500 in the total amount of $143.61. MOTION CARRIED UNANIMOUSLY. 3-0. 2. James Monnett a. Regence Explanation of Benefits Statement No. 0034600055400 processed on December 20, 2000, for services received on November 27 and 28, 2000, in the amount of $28.25. Motion by Member Larson, second by Member Brothers to approve Mr. Monnett's claim in the amount of $28.25. MOTION CARRIED UNANIMOUSLY. 3-0. 3. Sammy Mulkey a. 2000 Form SSA-1099 Benefit Statement for reimbursement of Medicare premiums paid in the amount of $546.00. Motion by Member Larson, second by Member Brothers to approve reimbursement of Medicare premiums paid in the year 2000 in the amount of $546.00. MOTION CARRIED UNANIMOUSLY. 3-0. It was consensus of the Board members to authorize the secretary to process for payment all future claims for payment for reimbursement of Medicare premiums paid, unless unusual circumstances occur. Regence Explanation of Benefits Statement No. 0101110389100 processed on January 24, 2001, for services received on December 27, 2000, in the amount of $62.00 (Claim for Payment says $61.90). Motion by Member Larson, second by Member Brothers to approve Mr. Pitzl's claim in the amount of $61.90. MOTION CARRIED UNANIMOUSLY. 3-0. V.
