CALL TO ORDER
Chair Jim Kelly called the meeting to order at 10:30 AM in the Council
Conference Room located at Auburn City Hall, 25 West Main Street, Auburn,
Washington. Chair Kelly and the following Board members were present: Chair
Pro Tem Bud Larson, Member Gene Cerino, and Member Russ Vandver.
Member Stacey Brothers was excused. Others present during the meeting
included: Associate City Attorney Joseph Beck, Human Resources/Risk
Management Director Brenda Heineman, and Board Secretary Danielle Daskam.
APPROVAL OF MINUTES
Minutes of the November 3, 2003 LEOFF Board Meeting
Motion by Chair Pro Tem Larson, second by Member Cerino to approve
the minutes as distributed.
MOTION CARRIED UNANIMOUSLY. 4-0.
UNFINISHED BUSINESS
Pending Disability Leave/Retirement Applications
There was no pending disability leave/retirement application.
B. Claims

tabled Mr. Petersen's claim pending receipt of additional
documentation to support the medical necessity for the contact

lenses and the type of contact lenses prescribed. No new
information was received.
Motion by Chair Pro Tem Larson, second by Member Cerino to
deny the claim due to lack of requested information.
MOTION CARRIED UNANIMOUSLY. 4-0.
William Pitzl
Request for reimbursement of medical device expense incurred on
July 24, 2003 in the amount of $12.00.
On November 3, 2003, the Board tabled Mr. Pitzl's claim pending
the receipt of an insurance Explanation of Benefits Statement. No
new information was received.
Motion by Member Cerino, second by Member Vandver to table the
claim until the next meeting pending receipt of additional
information.
MOTION CARRIED UNANIMOUSLY. 4-0.

Claim for payment for reimbursement of vision expenses
follows:

LEOFF Board Policy 9.02(A)(2) provides for reimbursement for
lenses and frames as follows:
Frames: $ 90.00
Bifocal Lenses $ 60.00
On November 3, 2003, the Board tabled Mr. Wilson's claim
pending the receipt of a signed vision claim worksheet. Mr. Wilson
submitted a signed worksheet enclosed in the agenda packet.
Motion by Chair Pro Tem Larson, second by Member Vandver to
pay the claim in accordance with LEOFF Board policy in the
amount of $90.00 for frames and $60.00 for lenses.
MOTION CARRIED UNANIMOUSLY. 4-0.

Motion by Member Vandver, second by Chair Pro Tem Larson to
approve the claim in the amount of $10.00.
MOTION CARRIED UNANIMOUSLY. 4-0.
Correspondence
No correspondence was received.
Discussion
Policy and Procedure Manual Review
Chair Kelly noted that the Board is required to review the policies
and procedures manual in even numbered years beginning in
January. The following sections were identified for possible
amendment:
Section 1.05, Elections
Section 1.04, Meeting (update meeting time to reflect 10:30 AM)
Section 9.02, Vision Benefits
Chair Kelly requested the Board Secretary survey corrective vision
lens and frames costs within the Auburn area.
Chair Pro Tem Larson requested the Board consider adding a
provision for medically necessary prescription sunglasses.
Chair Kelly requested the Board members review Section 9.03,
relating to hearing aids and the Board's designation of Costco as
its preferred hearing aid provider.
Other Business
Chair Kelly thanked Members Cerino and Brothers for their service on the
Board. Member Cerino announced that Councilmembers-elect Peloza
and Thordarson will be the new Council representatives on the LEOFF
Board beginning in January 2004.
