Inc. (hereinafter referred to as he "CONSULTANT"), as an Amendment to the Agreement between the parties for Agreement No. AG-C-182 and f:xecuted on the 9th day of May 2002. and amended by agreement dated the 13th day of August 2003. The changes to the agreement are described as follows: I. CONTRACT TERM: The term of the Agreement for Professional Services is extended to December 31. 2004. 2. SCOPE OF WORK: There is no change in the scope of work. 3. COMPENSATION: There is no change to the amount authorized in the original task order and amendment #1 for a combined total of$66,073.00. REMAINING TERMS UNCHANGED: That all other provisions of the Agreemf:nt between the parties for AG-C-182 executed on the 9th day of May 2002, shall remain unchanged, and in full force and effect. (hereinafter referred to as the "CONSULT ANT"), as an Amendment to the Agreement between the parties for AG-C-182 executed on the 9th day of May 2002 and amended by agreement dated the 13th day of August 2003, and amended by agreement dated the 27th day of January 2004. The changes to the agreement are described as follows: 1. CONTRACT TERM: The term of the Agreement for Professional Services is extended to May 31, 2005. 2. SCOPE OF WORK: See Exhibit A, attached and by this reference made part of this Agreement. 3. COMPENSATION: The amount of this amendment is $4,525.00. See Exhibit B, attached and by this reference made part of this Agreement. The total contract amount is increased to a total of $70,598.00. There is no change to the Management Reserve Funds. REMAINING TERMS UNCHANGED: That all other provisions of the Agreement between the parties for AG-C-182 executed on the 9th day of May 2002, and amended by agreement dated the 13th day of August 2003, and amended by agreement dated the 27th day of January 2004, shall remain unchanged, and in full force and effect. Page 2 of2 EXHIBIT A - Scope of Work INTRODUCTION The City wishes to supplement Perteet's scope of work for ClO06 3rd Slreel SE/Cross Street SE Road Improvement Project by modifying the signal plan for the A Street/3rd Street intersection. The supplement is necessary due to base map revisions made by the City. These revisions include reduction of the curb radius in the southwest comer and deletion of the crosswalk on the south leg of A Street. The work under this supplement will be accomplished under the following elements of work: SCOPE OF SERVICES A. Project Management and Coordination I. Monitor tasks and budget. 2. QAlQC. B. Signal Design I. Update base map for signal plan. 2. Modify signal pole layout to accommodate revised curb return in southwest comer of A Street/3rd Street intersection. 3. Revise loop locations on west leg oOrd Street and south leg of A Street SE. 4. Delete pedestrian signals for deleted crosswalk on south leg of A Street. 5. Modify conduit and wiring related to the above revisions. ITEMS TO BE FURNISHED BY THE CITY I. Revised base map in electronic AutoCAD 2002 format and a hard copy. ITEMS TO BE FURNISHED BY THE CONSULTANT I. One (1) electronic copy of the revised signal revision in AutoCAD 2002 for review. ASSUMPTIONS Any review comments made by the City will be included prior to making a final submittal of any signal plans.
