MINUTES
I. CALL TO ORDER
Chair Jim Kelly called the meeting to order at 10:00 a.m. in the Council
Conference Room located at Auburn City Hall, 25 West Main Street in Auburn.
Chair Kelly and the following Board members were present: Chair Pro Tem Bud
Larson, Member Roger Thordarson, Member Lynn Norman and Member Bill
Petersen. Also present were Board Attorney Joseph Beck and Board Secretary
Danielle Daskam.
II. APPROVAL OF MINUTES
A. Minutes of the August 7, 2007 LEOFF Board Meeting
Motion by Member Thordarson, second by Member Petersen to approve
the August 7, 2007 minutes as distributed.
MOTION CARRIED UNANIMOUSLY. 5-0.
III. UNFINISHED BUSINESS
A. Pending Disability Leave/Retirement Applications
1. Rich Schneider
Effective Date of Disability: July 7, 2007
Last Day of Service: July 6, 2007
On July 3, 2007, the LEOFF Board approved disability leave for
Rich Schneider effective July 7, 2007.
Mr. Schneider's monthly physician report dated August 30, 2007
from Dr. James Wagner was distributed at the meeting.
Motion by Member Petersen, second by Chair Pro Tem Larson to
continue disability leave for an additional month or until cessation
of disability, whichever occurs first.
MOTION CARRIED UNANIMOUSLY. 5-0.
B. Medical/Hearing/Vision Claims

Request for reimbursement of medical expenses in the total
amount of $372.00 per Regence Explanation of Benefits Statement
#0713695025100 for services received April 13, 2007.
On July 3 and August 7, 2007, the LEOFF Board tabled Mr.
Ogden's claim pending the receipt of the Medicare explanation of
benefits statement. No new information was received.
Motion by Member Petersen, second by Chair Pro Tem Larson to
table the claim for an additional month pending the receipt of the
information requested.
MOTION CARRIED UNANIMOUSLY. 5-0.
2. Don Casselman
Request for reimbursement of medical expenses received on June
5, 2007 in the amount of $22.40.
On August 7, 2007, the Board tabled Mr. Casselman's claim
pending receipt of additional information in the form of an
insurance explanation of benefits statement. No new information
was received.

3. William Pitzl
Request for reimbursement of medical expenses in the amount of
$1,311.25 for medical services received on May 2, 2007.
On August 7, 2007, the Board tabled Mr. Pitzl's claim pending the
receipt of additional information in the form of an insurance
explanation of benefits statement. Subsequently, Mr. Pitzl
submitted the applicable Regence Explanation of Benefits
Statement.
Motion by Member Petersen, second by Chair Pro Tem Larson to
approve the claim in the amount of $1,311.25.

IV. NEW BUSINESS
A. Disability Leave/Retirement Applications
No new application for disability leave/retirement was received.
B. Appeals/Requests for Reconsideration
1. Roger H. Lee
Request for reconsideration of Mr. Lee's claim for reimbursement
of dental expenses in the amount of $2,444.10.
On July 3, 2007, the LEOFF Board approved a portion of Mr. Lee's
claim up to the annual benefit level. Mr. Lee was advised that in
order for the Board to consider the balance of his claim, Mr. Lee
would be required to submit specific information from his physician
on Mr. Lee's medical condition and how the dental treatment, or
lack thereof, would affect his existing medical condition. In
response to the Board's request, Mr. Lee submitted a letter from
his treating dentist dated August 7, 2007, outlining the medical
necessity of the dental procedures completed.
Motion by Member Petersen, second by Chair Pro Tem Larson to
approve the claim in the amount of $2,444.10.
MOTION CARRIED UNANIMOUSLY. 5-0.
C. Medical/Hearing/Vision Claims
1. Mick Crawford
Request for reimbursement of vision expenses in the amount of
$298.00.
Mr. Crawford had not received reimbursement for vision expenses
in the last year.
Motion by Member Thordarson, second by Chair Pro Tem Larson
to approve the claim in the amount of $225.00.
MOTION CARRIED UNANIMOUSLY. 5-0.
2. William Pitzl
Request for reimbursement of dental expenses in the amount of
$568.50.

Mr. Pitzl had not received reimbursement for dental expenses in
2007.
Motion by Member Petersen, second by Chair Pro Tem Larson to
approve the claim in the amount of $568.50.
MOTION CARRIED UNANIMOUSLY. 5-0.
3. James Kelly
Request for reimbursement of Lasik eye surgery expenses in the
amount of $725.00.
Chair Kelly recused himself from discussion and consideration of
this claim.
Motion by Member Petersen, second by Member Thordarson to
approve the claim in the amount of $725.00.
MOTION CARRIED. 4-0.
4. Richard Schneider
Request for reimbursement of medical expenses in the amount of
$1,262.10 for medical services received on May 4, 2007 per
Regence Explanation of Benefits Statement #0713032483300.
Motion by Member Petersen, second by Chair Pro Tem Larson to
approve the claim in the amount of $1,262.10.
MOTION CARRIED UNANIMOUSLY. 5-0.
5. John Burgess
Request for reimbursement of vision expenses in the amount of
$522.00.
Mr. Burgess had not received reimbursement for vision expenses in
the last year.
Motion by Chair Pro Tem Larson, second by Member Thordarson
to approve the claim in the amount of $225.00.
MOTION CARRIED UNANIMOUSLY. 5-0.

Request for reimbursement of medical expenses in the amount of
$41.43 for medical services received on May 26, 2007 per
Regence Explanation of Benefits Statement #0717932677900;
$28.34 for medical services received on May 23, 2007 per
Regence Explanation of Benefits Statement #0717632890200; and
$31.62 for medical services received on March 23, 2007 per
Regence Explanation of Benefits Statement #0717632890000 for a
total claim of $101.39.
Motion by Member Petersen, second by Chair Pro Tem Larson to
approve the claims in the total amount of $101.39.
MOTION CARRIED UNANIMOUSLY. 5-0.
7. Robert S. Wigley
Request for pre-approval to purchase hearing aids from Costco
Hearing Aid Center in the total amount of $2798.00. Mr. Wigley
had not received payment for hearing aids in the past.
Motion by Member Petersen, second by Chair Pro Tem Larson to
approve the purchase of two hearing aids for Mr. Wigley in the total
amount of $2,798.00.
MOTION CARRIED UNANIMOUSLY. 5-0.
D. Correspondence
No correspondence was received.
E. Discussion
There was no other discussion.
V. ADJOURNMENT
There being no further business to come before the Board, the meeting
adjourned at 10:18 a.m.
