MINUTES
I. CALL TO ORDER
Chair Jim Kelly called the meeting to order at 10:00 a.m. in the Council
Conference Room located at Auburn City Hall, 25 West Main Street in Auburn.
Chair Kelly and the following Board members were present: Member Roger
Thordarson, Member Lynn Norman and Member Bill Petersen. Chair Pro Tem
Larson was excused. Also present were Board Attorney Joseph Beck and Board
Secretary Danielle Daskam.
II. APPROVAL OF MINUTES
A. Minutes of the July 3, 2007 LEOFF Board Meeting
Motion by Member Thordarson, second by Member Petersen to approve
the July 3, 2007 minutes as distributed.
MOTION CARRIED. 4-0.
III. UNFINISHED BUSINESS
A. Pending Disability Leave/Retirement Applications
1. Rich Schneider
Effective Date of Disability: July 7, 2007
Last Day of Service: July 6, 2007
On July 3, 2007, the LEOFF Board approved disability leave for
Rich Schneider effective July 7, 2007.
Mr. Schneider's monthly physician's report dated August 6, 2007
from Dr. Gilbert Johnson was distributed at the meeting.
Motion by Member Petersen, second by Member Norman to
continue disability leave for an additional month or until cessation
of disability, whichever occurs first.
MOTION CARRIED. 4-0.
B. Medical/Hearing/Vision Claims

Request for reimbursement of medical expenses in the total
amount of $285.00 per Regence Explanation of Benefits Statement
#0704795031800 and Medicare claim #11-07032-099-290 for
medical services received on January 29, 2007.
On May 1, June 5 and July 3, 2007, the Board tabled Mr. Lee's
claim pending the receipt of additional information regarding the
medical necessity of the medical services provided. No new
information was received.
Motion by Member Petersen, second by Member Thordarson to
deny the claim due to lack of information.
MOTION CARRIED. 4-0.
2. Ray Ogden
Request for reimbursement of medical expenses in the total
amount of $372.00 per Regence Explanation of Benefits Statement
#0713695025100 for services received April 13, 2007.
On July 3, 2007, the LEOFF Board tabled Mr. Ogden's claim
pending the receipt of the Medicare explanation of benefits
statement. No new information was received.
Motion by Member Thordarson, second by Member Petersen to
table the claim for an additional month pending the receipt of the
information requested.
MOTION CARRIED. 4-0.
IV. NEW BUSINESS
A. Disability Leave/Retirement Applications
No new application for disability leave/retirement was received.
B. Appeals/Requests for Reconsideration
No appeal or request for reconsideration was filed.
C. Medical/Hearing/Vision Claims
1. David Schuur
Requests for reimbursement of dental expenses in the amount of
$385.00 and $74.10.

Motion by Member Thordarson, second by Member Petersen to
approve the claim in the amount of $225.00.
MOTION CARRIED. 4-0.
3. David Hannity
Request for reimbursement of medical expenses in the total
amount of $151.00 per Regence Explanation of Benefits Statement
#07121328112500 for services received April 27, 2007.
Motion by Member Thordarson, second by Member Petersen to
deny the claim finding the claim was for routine preventative
services.
MOTION CARRIED. 4-0.
4. William Pitzl
Request for reimbursement of medical expenses in the amount of
$1,311.25 for medical services received on May 2, 2007.
Chair Kelly noted that no Regence Explanation of Benefits
statement was submitted with the claim.
Motion by Member Thordarson, second by Member Petersen to
table the claim pending the receipt of additional information in the
form of an insurance explanation of benefits statement.

5. Don Casselman
Request for reimbursement of medical expenses received on June
5, 2007 in the amount of $22.40.
Mr. Casselman's claim was added to the agenda. Chair Kelly
noted that no insurance explanation of benefits statement was
submitted with the claim.
Motion by Member Thordarson, second by Member Petersen to
table the claim pending receipt of additional information in the form
of an insurance explanation of benefits statement.
MOTION CARRIED. 4-0.
6. Bruce Caldwell
Request for reimbursement of medical expenses in the amount of
$41.43 per Regence Explanation of Benefits Statement
#0715032456200 for services received on April 26, 2007.
Mr. Caldwell's claim was added to the agenda.
Motion by Member Thordarson, second by Member Petersen to
approve the claim in the amount of $41.43.
MOTION CARRIED. 4-0.
D. Correspondence
No correspondence was received.
E. Discussion
