CALL TO ORDER Chair Pro Tem Bud Larson called the meeting to order at 10:00 AM in Conference Room I located at Auburn City Hall, 25 West Main Street, Auburn, WA. Members present included: Chair Pro Tem Bud Larson, Member Jeanne Barber, and Member Stacey Brothers. Chair Jim Kelly and Member Russ Vandver were excused from the meeting. Others present included Mayor Charles A. Booth and Associate City Attorney Karen Gulliver. I1. APPROVAL OF MINUTES A. Minutes of the July 2, 2001 LEOFF Board Meeting Motion by Member Brothers, second by Member Barber to approve the July 2, 2001 LEOFF Board minutes as distributed. MOTION CARRIED UNANIMOUSLY. 3-0. II1. UNFINISHED BUSINESS A. Pending Disability I eave/Retirement Applications 1. Harold F. Philbin Effective Date of Disability: April 1, 2001 Last Day of Service: March 31, 2001 On April 2, 2001, the LEOFF Board approved disability leave for Mr. Philbin effective April 1, 2001 with last day of service on March 31, 2001. On June 4, 2001, the Board waived the monthly physician's report, with the understanding that Mr. Philbin would be subject to review by the Board's physician at a time in the future. The Board's waiver of monthly physician reports was based on the May 2, 2001 letter from Ron Kuppersmith MD. Mr. Philbin's six months disability leave will conclude on September 31, 2001 LEOFF Board Minutes August 6, 2001 Page 1 City of Auburn Law Enforcement Officers and Firefighters Disability Board Monday, August 6, 2001 - 10:00 AM - Conference Room 1 Motion by Member Barber, second by Member Brothers to refer Mr. Philbin to the Board's physician for examination in order to determine his eligibility for disability retirement; and to continue Mr. Philbin's disability leave for an additional month, or until cessation of disability, whichever occurs first. MOTION CARRIED UNANIMOUSLY. 3-0. B. Claims 1. John Wickstrom a. Request dated March 30, 2001 for reimbursement of lenses expense in the amount of $104.00. On May 7 and June 4, 2001, the LEOFF Board tabled Mr. Wickstrom's claim for reimbursement of lenses expense pending the receipt of information confirming that Mr. Wickstrom had submitted his claim to the insurance company, Vision Service Plan. On June 7, 2001, the LEOFF Board received the enclosed Eye Exam and Corrective Lenses Worksheet completed by his provider. On July 2, 2001, the LEOFF Board tabled Mr. Wickstrom's claim to allow time for Chair Kelly to contact Mr. Wickstrom regarding his claim. Chair Pro Tem Larson recommended that the Board continue Mr. Wickstrom's claim for an additional month pending confirmation that Chair Kelly contacted Mr. Wickstrom. Motion by Member Brothers, second by Member Barber to table Mr. Wickstrom's claim until the next regular meeting. MOTION CARRIED UNANIMOUSLY. 3-0. Regence Explanation of Benefits Statement No. 0101700238600 processed on January 31, 2001 for services received on January 11, 2001 in the amount of $35.00. On May 7, June 4, and July 2, 2001, the LEOFF Board tabled Ms. Darough's claim pending submission of her claim to Medicare as primary insurer and Regence as secondary. No new information was received. Motion by Member Barber, second by Member Brothers to deny Ms. Darough's claim due to lack of documentation and advise Ms. Darough that she can resubmit her claim at a later date with the requested additional information. MOTION CARRIED UNANIMOUSLY. 3-0. 3. John A. Baker a. Request for reimbursement of medical expenses incurred in March 2001 in the amount of $62.00. On July 2, 2001, the Board tabled Mr. Baker's claim pending the receipt of the appropriate insurance Explanation of Benefits Statement(s). No new information was received. Motion by Member Barber, second by Member Brothers to table Mr. Baker's claim until the next regular meeting. MOTION CARRIED UNANIMOUSLY. 3-0. IV. NEW BUSINESS A. Disability Leave/Retirement Applications There was no new application for disability leave/retirement. B. Appeals No request for reconsideration or appeal was received. Fuller a. Regence Explanation of Benefits Statement No. 0115020541000 procesed on July 2, 2001 for services received on March 6, 2001 in the amount of $53.36; Regence Explanation of Benefits Statement No. 0115020540800 processed on July 2, 2001 for services received on April 6, 2001 in the amount of $93.36; and Regence Explanation of Benefits Statement No. 0115020540900 processed on July 2, 2001 in the amount of $53.36 for a total claim of $200.08. Motion by Member Barber, second by Member Brothers to approve Mr. Fuller's claim in the total amount of $200.08. MOTION CARRIED UNANIMOUSLY. 3-0. b. Regence Explanation of Benefits Statement No. 0118121232100 processed on July 16, 2001 for services received on June 6, 2001 in the amount of $53.36. Motion by Member Brothers, second by Member Barber to approve Mr. Fuller's claim in the amount of $53.36. MOTION CARRIED UNANIMOUSLY. 3-0. 2. Ronald L. Cude a. Request for reimbursement of medical services received on March 9, 2001 in the amount of $202.47. (No Explanation of Benefits Statement included.) b. Request for reimbursement of medical services received in March, April and May 2001 in the amount of $354.79. (No Explanation of Benefits Statement included.) Motion by Member Barber, second by Member Brothers to table Mr. Cude's claims pending receipt of the required insurance explanation of benefits statements. MOTION CARRIED UNANIMOUSLY. 3-0. 3. James Fisk a. Regence Explanation of Benefits Statement No. 0034631525700 processed on December 20, 2000 for services received on November 8, 2000 in the amount of $350.00. Chair Pro Tem Larson noted that Mr. Fisk's claim was processed by insurance on December 20, 2000, which is beyond the Board's ninety (90) day limit for filing claims. Motion by Member Barber, second by Member Brothers to reject Mr. Fisk's claim as the claim is beyond the ninety day limit for filing claims. MOTION CARRIED UNANIMOUSLY. 3-0. D.
