MINUTES
1. CALL TO ORDER
Chair Jim Kelly called the meeting to order at 2:00 p.m. in the Council
Conference Room located at Auburn City Hall, 25 West Main Street in Auburn.
Chair Kelly and the following Board members were present: Member Bill
Petersen, Member Virginia Haugen, and Member Lynn Norman. Chair Pro Tern
Bud Larson (member-at-large) was excused.
II. APPROVAL OF MINUTES
A. Minutes of the February 5, 2008 LEOFF Board Meeting
Motion by Member Petersen, second by Member Norman to approve the
February 5, 2008 minutes as distributed.
MOTION CARRIED. 4-0.
III. UNFINISHED BUSINESS
There is no unfinished business.
IV. NEW BUSINESS
A. Disability Leave/Retirement Applications
No new application for disability leave/retirement was received.
B. Appeals/Requests for Reconsideration
No request for reconsideration was received.
C. Medical/Hearing/Vision Claims
1. Loren Anderson
Request for reimbursement of dental expenses in the amount of
$1,005.00. Mr. Anderson has not received reimbursement for
dental expenses in 2008.
Motion by Member Petersen, second by Member Norman to
approve the dental claim in the amount of $1,005.00.
MOTION CARRIED. 4-0.

2. David Fugit
Request for reimbursement of dental expenses in the amount of
$297.35. Mr. Fugit has not received reimbursement for dental
expenses in 2008.
Motion by Member Petersen, second by Member Norman to
approve the dental claim in the amount of $297.35.
MOTION CARRIED. 4-0.
3. Don Webb
Request for reimbursement of dental expenses in the amount of
$200.00. Mr. Webb has not received reimbursement for dental
expenses in 2008.
Motion by Member Petersen, second by Member Norman to
approve the dental claim in the amount of $200.00.
MOTION CARRIED. 4-0.
An additional request for reimbursement of dental expenses from
Mr. Webb in the amount of $895.00 was distributed prior to the
meeting.
Motion by Member Petersen, second by Member Norman to
approve the dental claim in the amount of $895.00.
MOTION CARRIED. 4-0.
(Action on this item took place after Item IV.C.5 of these minutes.)
4. Dale Volstad
Request for reimbursement of dental expenses in the amount of
$1,200.00. Mr. Volstad has not received reimbursement for dental
expenses in 2008.
Motion by Member Petersen, second by Member Norman to
approve the dental claim in the amount of $1,200.00.
MOTION CARRIED. 4-0.

5. Roger H. Lee
Request for reimbursement of dental expenses in the amount of
$1,264.45. Mr. Lee has not received reimbursement for dental
expenses in 2008. (September and October 2007 expenses were
found medically necessary.)
Motion by Member Petersen, second by Member Norman to
approve the dental claim in the amount of $1,264.45.
Chair Kelly noted that dental services received by Mr. Lee
appeared to be of medical necessity.
MOTION CARRIED. 4-0.
D. Correspondence
No correspondence was received.
E. Policy and Procedure Manual Review
Board members briefly reviewed the vision cost survey.
Chair Kelly requested that Board members review the Policy and
Procedure Manual and bring forth suggestions for possible amendments
to the next LEOFF Board meeting.
V. ADJOURNMENT
There being no further business to come before the Board, the meeting
adjourned at 2:10 p.m.
